MOUNJARO® 12.5mg KwikPen® is a multi-dose pre-filled injection pen designed to administer 12.5 mg tirzepatide in 0.6 mL through a subcutaneous injection once weekly. It is most often used as an intermediate titration dose for adults being treated for type 2 diabetes mellitus or for weight management in patients with obesity or overweight and related comorbidities. Each KwikPen® contains 2.4 mL of solution with a total of 50 mg tirzepatide, providing four doses of 12.5 mg. The solution is clear, colourless to slightly yellow and is intended for use in the abdomen, thigh, or upper arm.

MOUNJARO® is indicated for adults with insufficiently controlled type 2 diabetes mellitus as an adjunct to diet and exercise, either as monotherapy or in combination with other antidiabetic medicines. It is also approved for weight management, including weight reduction and maintenance, in adults with a BMI ≥30 kg/m², or BMI ≥27 kg/m² when accompanied by at least one weight-related condition such as hypertension, cardiovascular disease, dyslipidaemia, obstructive sleep apnoea, or prediabetes.

• Indications:

  • Type 2 diabetes mellitus treatment in adults

  • Obesity management (BMI ≥30 kg/m²)

  • Overweight patients (BMI ≥27 kg/m²) with comorbidities

• Contraindications:

  • Known hypersensitivity to tirzepatide or excipients

  • Not recommended in patients with severe gastrointestinal disease (e.g., gastroparesis)

  • Use with caution in those with a history of pancreatitis

• Side effects:

  • Very common: Nausea, vomiting, diarrhoea, constipation, abdominal pain

  • Common: Decreased appetite, dyspepsia, fatigue, mild injection-site reactions

  • Occasional: Gallbladder-related events (e.g., cholelithiasis, cholecystitis)

  • Rare: Acute pancreatitis, severe hypersensitivity reactions

  • Laboratory findings: Increases in amylase and lipase levels

Administration of MOUNJARO® requires proper injection technique and adherence to weekly dosing. Patients can self-administer after appropriate training. Each dose should be injected into the abdomen, thigh, or upper arm, with injection sites rotated to prevent irritation.

• Dosing schedule:

  • Start with 2.5 mg weekly for 4 weeks, then escalate gradually

  • 12.5 mg is typically used as a step before reaching the maximum 15 mg dose

  • Maximum dose: 15 mg once weekly

• Missed dose:

  • If within 4 days: inject as soon as possible

  • If more than 4 days: skip and resume on the scheduled day

• Injection steps:

  • Attach a new sterile needle before use

  • Dial to 1 in the window (=0.6 mL dose)

  • Inject subcutaneously, hold for 5 seconds until window shows 0

• Storage requirements:

  • Unopened pens: Refrigerate at 2–8 °C

  • Do not freeze; discard if frozen

  • In-use pens: May be stored at ≤30 °C for up to 30 days

  • Discard after 30 days, even if solution remains

  • Keep out of sight and reach of children
